[How to apply] At the Ayase Seiryu-kan in Kuwabukuro Biotope Park or, send fax or a double postal card toKuwabukuro Biotope Park, in which state clearly All those who wish to participate (including guardians)Please provide the following information: ① Address, ② Name (with phonetic spelling), ③ Age/Grade, ④ Telephone/Fax number, ⑤ Preferred date (up to 2 choices), and "I would like to participate in the night adventure.Please clearly state "[this]" when applying (if using a return postcard, write the recipient's name on the return side).
